Nominations
Each year, we invite the community to nominate an individual they thing would make a good role model for others more currently diagnosed with breast cancer. 12 honorees are chosen to be represented on our annual Breast Cancer Survivors Calendar for the upcoming year. Nominees must be at least three years post op and out of treatment.
Once the selections are made, Edda Taylor, Edda Taylor Photographie, invites each of the honorees to her studio in Crown Point for a personal photo shoot. Jim Pellegrini, the Times Media Company, coordinates the calendar with Edda, which includes a biography of each honoree highlighting their journey from diagnosis to survivorship.
Nomination Process
Submission Address: If you would like to nominate a man or woman who has survived the diagnosis of breast cancer, please fill out and submit the nomination form. You may also print out the form, and mail the completed form to:

Required Information: Include the nominee's name, contact information, survivorship duration, and a brief story of their journey.
Eligibility Criteria
Doors Open: 10:30 am.
Activities: Shopping, raffle viewing, and visiting opportunities.
Ticket Information
Survivorship: Must be a breast cancer survivor for at least three years.
Treatment Status: Out of active treatment at the time of nomination.
